      Please use this form when applying to colleges. Please fill out the form completely, attach the fee and return to Mrs. Chiarello in the Guidance Office. Please submit at least 10 working days before the due date!

      Please use this form when applying for scholarships. Transcripts for scholarships are FREE! Please submit at least ten working days before the due date!
    • Transcript Release Form for College Coaches
      With this release signed by the parent fo the Notre Dame student-athlete, the Guidance Office may respond to requests for transcripts by college coaches.
