  • Exclusion Policy

    It is our goal to keep your child in school; however, if he/she has a fever, is vomiting or has an illness or injury that we feel should be evaluated by a physician, you will be asked to pick up your child.
  • Physicals

    It is the policy of Notre Dame that all freshmen and transfer students have a physical before entering school. This physical is also good for sports and is in effect for 365 days. For example, if your child has a physical in the summer, this physical is good for all three seasons of sports. However, for every sport a new parent consent form, parent questionnaire and emergency form must be completed. Physical forms are available as a download from this page.   • Medications

    Without a doctor's order, the school nurse cannot administer any medications, including over-the-counter medications. If your child has a condition that warrants medication, please have the physician complete a Medication Order Form. Medication Forms are available as downloads from this page. Students are not permitted to carry medication or give medication to anyone in school. The exception to this rule is for those students who require inhalers or epipens. The state of New Jersey does allow those students to self carry as long as we have a Self Administration order form from the physician on file. These forms are also avaiable as downloads.
